Implementation science seeks to close the gap between evidence and practice, or between what we know and what is actually done. NCCOR’s Implementation Scorecard was informed by the experiences and challenges faced by practitioners implementing childhood nutrition, physical activity, and obesity prevention interventions. The tool is designed to help practitioners spot strengths and blind spots as they move through the four phases of implementing new interventions or programs: Exploration, Preparation, Implementation, and Sustainment. The accompanying case study illustrates how a non-profit focused on community nutrition utilizes the scorecard to adapt and implement an existing evidence-based meal delivery program in their school district.
